Key facts and official sources

  • The Arizona Department of Health Services (ADHS) is the CLIA state survey agency for Arizona. See the CMS state agency contacts.

  • Arizona labs apply for CLIA with Form CMS-116, submitted to the state agency.

  • Federal CLIA requirements are in 42 CFR Part 493. They apply the same way in Arizona as in other non-exempt states.

What does a CLIA consultant do for Arizona labs?

  • New labs: certificate selection, the CMS-116, test menu planning, method verification and SOPs.

  • Survey readiness: mock inspections against Part 493 or your accreditor's standards.

  • After a survey: CMS-2567 plans of correction and evidence packages (POC examples).

  • PT failures: root cause analysis, corrective action and reinstatement planning (PT failure guide).

  • Personnel: CLIA lab director and technical consultant services, and personnel file audits.

  • Ongoing quality: QC, competency and quality assessment programs.

How do Arizona labs apply for CLIA?

Choose the certificate that matches your most complex test (Waiver, PPM, Compliance or Accreditation). Complete Form CMS-116 and submit it to ADHS as the state agency. Pay the CLIA fee once CMS bills you. Do not test patient samples for nonwaived tests until the certificate is in place. See our CMS-116 guide.

Which Arizona labs do we work with?

Physician office labs, urgent care and multi-site clinics, skilled nursing and long-term care facilities, specialty practices, and independent and reference labs. Arizona labs face the same federal survey focus areas as labs elsewhere: proficiency testing, lab director oversight, QC and competency.

Do you work on site in Arizona?

Yes. Chandler is in the Phoenix metro, so on-site visits across the Valley are easy to schedule. We plan visits to Tucson, Flagstaff and other Arizona cities as each engagement requires. Much of the work, such as document review, POC drafting and remote director oversight between required visits, can be done remotely.

What happens after an Arizona CLIA survey?

ADHS sends the Statement of Deficiencies (Form CMS-2567). You submit a plan of correction by the deadline in the cover letter. Condition-level findings can lead to sanctions under 42 CFR 493 Subpart R, so treat every deadline as firm.

Frequently asked questions

Can you serve as our Arizona lab director?

We provide CLIA lab director services where a qualified director is available for your test complexity and specialties. Directors must meet 42 CFR 493 Subpart M and the on-site visit requirements.

Who is the CLIA state agency in Arizona?

The Arizona Department of Health Services. CMS lists the current contact on its state agency contacts page.

Does Arizona require a state lab license in addition to CLIA?

Requirements can change, so confirm current state licensing directly with the Arizona Department of Health Services (ADHS). We can help you work out what applies to your lab.

Do you only work in the Phoenix area?

No. We serve labs statewide and in all 50 states.
