Laboratory Quality Management System (QMS) for CLIA Labs
A laboratory quality management system (QMS) is the documented set of policies, SOPs, records and reviews a lab uses to produce reliable results. In a CLIA lab, it ties together quality control or an IQCP, proficiency testing, method verification, competency assessment and quality assessment. CLSI organizes it into 12 quality system essentials.
Key facts and official sources
CLIA quality system requirements for nonwaived testing are in 42 CFR 493 Subpart K, which covers preanalytic, analytic and postanalytic systems.
CLSI's QMS framework is described in CLSI QMS01.
PT requirements are in Subpart H.

How SLC helps
SLC builds and maintains quality management systems for CLIA labs. We develop Individualized Quality Control Plans (IQCP), SOPs, instrument validations and proficiency testing programs, and we prepare labs for CAP, COLA and Joint Commission inspections.
FAQs
Does CLIA require a formal QMS? CLIA requires the quality systems in Subpart K. A QMS is the structured way to meet and document them.
Are the CLSI QSEs mandatory? No. They are a voluntary framework, but many labs use them to organize CLIA and accreditation requirements.
How does IQCP fit into a QMS? An IQCP is a QC option for eligible tests. It is based on a risk assessment, a QC plan and quality assessment.
What records does a QMS need? QC, PT, verification, competency, maintenance, corrective action and quality assessment records. See record retention.
Do waived labs need a QMS? Waived labs must follow manufacturer instructions. A simple QMS still reduces errors.
How often should the QMS be reviewed? At least yearly, and after any significant error, survey finding or test change.
