Lab Revenue Cycle Management (Lab RCM)
Lab revenue cycle management (lab RCM) is the end-to-end financial process a diagnostic laboratory uses to get paid for the tests it performs, from capturing the test order through coding, claims and denials to final payment reconciliation. Sound Lab Consultants (SLC) is a nationwide firm that provides lab revenue cycle management and billing services alongside CLIA compliance.
Core Steps in the Lab Revenue Cycle
Order capture and registration: record the requisition, ordering practitioner, patient demographics and specimen details accurately at intake.
Eligibility and insurance verification: confirm active coverage and payer before or at the time of testing.
Medical necessity and ABN: check that diagnosis codes support coverage, and issue an Advance Beneficiary Notice (ABN) to Medicare patients when a test may not be covered.
Coding: assign CPT or PLA test codes, ICD-10-CM diagnosis codes and any required modifiers.
Charge capture: make sure every billable test performed becomes a charge, with nothing missed or duplicated.
Claim submission: send clean claims with the lab's CLIA number and payer-specific requirements.
Denial management: review denials and remittance codes, correct root causes and appeal where appropriate.
Payment posting and reconciliation: post payments and adjustments and reconcile them against expected reimbursement.
Reporting: track collections, denials and payer trends so issues are found early.
How lab RCM differs from general medical billing
Specialized test codes: labs bill many CPT laboratory codes, plus Proprietary Laboratory Analyses (PLA) codes for specific molecular and advanced tests. Medicare pays most lab tests under the Clinical Laboratory Fee Schedule (CLFS).
Coverage rules: many lab tests are governed by National Coverage Determinations (NCDs) and Local Coverage Determinations (LCDs) that list covered diagnoses (CMS coverage determination basics). See Medicare lab NCDs and LCDs.
CLIA number on every claim: CMS requires the lab's 10-character CLIA number on Medicare lab claims, in the 837P REF segment (qualifier X4) or item 23 of the paper CMS-1500 (CMS MLN006270; Claims Processing Manual Ch. 16).
QW modifier: certain waived tests must carry the QW modifier for Medicare to recognize them as CLIA-waived (CMS MM14569).
Date of service rules: lab-specific date-of-service policy applies, including the 14-day rule for some hospital specimens (CMS date of service policy).
How CLIA compliance affects reimbursement
Medicare pays only for tests that fall within the lab's CLIA certificate (CMS MLN006270). A lapsed certificate, a test outside the certificate's complexity level, or a missing or mismatched CLIA number can lead to denials. Keeping certificates, test menus and personnel compliant protects the revenue cycle. See CLIA consultant and CLIA certificate types.
Medicare CLFS and PAMA basics
Medicare pays most clinical lab tests under the CLFS, generally without patient cost-sharing. See Medicare CLFS.
Under PAMA, applicable laboratories report private payer rates that CMS uses to set CLFS rates (CMS PAMA reporting resources). See PAMA reporting for clinical laboratories.
For claim-level rules, see Medicare lab billing basics.

FAQ
What is lab revenue cycle management? Lab RCM is the full financial process a diagnostic lab uses to get paid for tests, from order capture through coding, claims, denials and payment reconciliation.
Why do lab claims need a CLIA number? CMS requires the lab's 10-character CLIA number on Medicare lab claims and pays only for tests covered by the lab's CLIA certificate.
What is the QW modifier? QW is a HCPCS modifier that identifies certain CLIA-waived tests on Medicare claims.
What are NCDs and LCDs? They are Medicare coverage policies, national and local, that define when a test is considered reasonable and necessary, often by diagnosis code.
Does Sound Lab Consultants help with lab billing? Yes. SLC provides laboratory revenue cycle management and billing services alongside CLIA compliance. See clinical lab billing.
