CAP Laboratory Consultant: Accreditation Readiness and Mock Inspections
A CAP laboratory consultant is an independent laboratory expert who helps a lab meet College of American Pathologists accreditation requirements by assessing it against the applicable CAP checklists and closing the gaps. The consultant prepares the lab for inspection and helps answer deficiencies, but the CAP alone decides accreditation.
Sound Lab Consultants (SLC) works with clinical, molecular and toxicology laboratories preparing for an initial CAP inspection, getting ready for a reinspection, or answering cited deficiencies. We support labs nationwide and are independent, not affiliated with or endorsed by the CAP.
What CAP Accreditation Involves
The CAP Laboratory Accreditation Program holds deeming authority from the Centers for Medicare & Medicaid Services, which allows a CAP inspection in lieu of a CMS inspection (cap.org). Per the CAP:
On-site inspections occur every two years using the CAP Accreditation Checklists.
Checklists are discipline-specific and updated annually.
Inspection teams are practicing laboratory professionals trained through a CAP inspector program.
CAP requirements commonly exceed CLIA, FDA and OSHA standards.
Because the checklists go beyond baseline CLIA rules, labs moving from a CLIA-only survey to CAP often find new documentation and oversight expectations. For broader background, see our guides to CLIA, CAP, COLA and TJC regulations and COLA vs CAP vs Joint Commission accreditation.
Mock Inspections Using CAP Checklists
A CAP mock inspection works the way an inspection does: we use the checklists for your disciplines, trace tests from order to report, review records, and talk with staff. The aim is to find what an inspector would find while you still have time to fix it.
Checklist selection based on your test menu and activity menu
Records review: QC, maintenance, temperature logs, PT, competency and personnel files
Test tracers from specimen receipt through result reporting
Staff walk-through of how to answer inspector questions and pull evidence
Written findings ranked by risk, with owners and target dates
Deficiency Remediation and Plans of Correction
When a CAP inspection cites deficiencies, the response has to show the root cause, the correction, and objective evidence that it is working. We help you:
Read each cited requirement and confirm what it actually asks for
Run root-cause analysis instead of patching the symptom
Draft corrective actions and gather evidence of correction
Set up monitoring so the fix holds through the next cycle

SOP and Quality Management
Inspectors check that procedures exist, are current, are approved, and match what staff actually do. We write and revise SOPs and build the quality system around them:
Procedure manuals with document control, review and approval records
Quality management plan, QC review and corrective-action workflows
Competency assessment programs and training records
QC strategy, including Westgard rules and IQCP where it fits
Lab Startups and CAP Readiness (Clinical, Molecular, Toxicology)
New labs that plan to pursue CAP accreditation do best when they build to the checklists from the start rather than retrofit later. We help:
Clinical labs: chemistry, hematology, immunology and other core areas, from CLIA certificate through CAP application
Molecular labs: workflow and contamination controls, validation packages and documentation for molecular checklist requirements
Toxicology labs: screening and confirmation workflows, chain-of-custody, and procedures for drug testing programs (see behavioral health drug testing labs)
Startup resources: start a clinical medical laboratory, new CLIA lab onboarding, and CLIA personnel qualifications.
Assay and Instrument Validation, Calibration Verification and PT Setup
Validation and verification records are among the first things inspectors ask for on new tests. We plan and document:
Method verification for FDA-cleared assays: accuracy, precision, reportable range and reference intervals
Validation for modified tests and laboratory-developed tests
Calibration verification and analytical measurement range checks on schedule
Instrument installation, comparisons and correlation studies (see our instrument experts)
Proficiency testing enrollment for each regulated analyte, result review and investigation of unacceptable results (see unsuccessful PT participation)
CAP vs COLA vs CMS State Survey
Every non-waived lab must meet CLIA. The difference is who inspects you and against which standard.
CAP
CLIA certificate: Certificate of Accreditation
Inspected by: Peer teams of practicing lab professionals
Standard: Discipline-specific CAP checklists, updated annually
On-site frequency: Every two years (per cap.org)
Commonly chosen by: Hospital, reference, pathology and high-complexity labs, or labs whose affiliated hospitals require CAP
COLA
CLIA certificate: Certificate of Accreditation
Inspected by: COLA surveyors
Standard: COLA criteria built on CLIA
On-site frequency: Two-year cycle
Commonly chosen by: Physician office labs, clinics and other labs wanting COLA's approach
CMS state survey
CLIA certificate: Certificate of Compliance
Inspected by: State agency on behalf of CMS
Standard: CLIA regulations and CMS interpretive guidelines
On-site frequency: Generally every two years
Commonly chosen by: Labs that do not elect an accreditor

Frequently Asked Questions
What does a CAP laboratory consultant do?
A CAP laboratory consultant helps a lab meet College of American Pathologists accreditation requirements by reviewing the lab against the applicable CAP checklists, fixing gaps, writing SOPs, supporting validations and helping respond to deficiencies. The consultant works independently of the CAP and does not grant accreditation.
What is a CAP mock inspection?
A practice review of your lab against the CAP checklists that apply to your test menu. It produces a prioritized list of gaps so you can correct them before the real inspection.
How often does the CAP inspect accredited labs?
According to the CAP, on-site inspections occur every two years using the CAP Accreditation Checklists.
Is SLC affiliated with the CAP?
No. Sound Lab Consultants is an independent consulting firm. We are not affiliated with or endorsed by the College of American Pathologists, and only the CAP makes accreditation decisions.
Can you help after we receive CAP deficiencies?
Yes. We help analyze each cited requirement, find the root cause, draft the corrective action and gather the evidence of correction your response needs.
Should my lab choose CAP, COLA or a CMS state survey?
It depends on your test menu, complexity, setting and what affiliated hospitals or payers expect. All three paths satisfy CLIA; we help you compare them before you apply.
Do you help new labs prepare for CAP accreditation?
Yes. We support clinical, molecular and toxicology lab startups with SOPs, validations, proficiency testing enrollment and a mock inspection before the initial CAP inspection.
Where can a lab get the CAP checklists?
The CAP says participating laboratories access the checklists through e-LAB Solutions Suite, while non-accredited laboratories may purchase them.
