Lab Manager vs Lab Director: Which Does Your Clinical Lab Need?
A lab director is the person legally responsible for a CLIA lab's overall operation and administration, and must meet the qualifications in 42 CFR Part 493. A lab manager runs daily operations and isn't a CLIA-defined role. Every nonwaived lab needs a qualified director. Many small labs don't need a full-time manager.
Side by side
Defined by CLIA?: Lab director: Yes (§§ 493.1405-1407, 1443-1445); Lab manager: No

Onsite requirement: Lab director: At least every 6 months, 4+ months apart, plus remote access; Lab manager: Usually daily (set by the employer)

Can be outsourced?: Lab director: Yes, through director services; Lab manager: The oversight part, yes. Daily bench work, no.
Checklist: which one are you hiring for?
☐ Do you lack a qualified CLIA director? Then you need a director, not a manager.
☐ Do you need someone to schedule staff and order supplies every day? That's a manager.
☐ Do you need technical consultant or supervisor coverage? Check the qualifications.
☐ Do you have open deficiencies? Think about compliance oversight.
Under 42 CFR 493.1407(c) (moderate complexity) and 493.1445(c) (high complexity), the laboratory director must be onsite at least once every 6 months, with at least 4 months between the two minimum visits, and must stay accessible to the lab for telephone or electronic consultation as needed. The director is responsible for the overall operation and administration of the lab, and stays responsible for every duty they delegate. One person may direct no more than five laboratories (§ 493.1407(d), § 493.1445(d)). Remote oversight does not replace the testing personnel, supervisors, or other people your lab's complexity and state require to be physically present.
Frequently asked questions
Is a lab manager the same as a lab director? No. The director is the CLIA-responsible person. The manager runs operations.
Does every lab need a director? Every CLIA-certified lab names a director. Waived labs have no CLIA director qualification requirements.
Can one person be both? Yes, if they meet the director qualifications for your complexity.
Who earns more? It varies. BLS groups managers under medical and health services managers (May 2025 median $123,860) and has no separate director category.
Can I outsource the director and keep an onsite lead? Yes, and it's a common setup for small labs. SLC provides lab director services.
What does the director delegate? Technical consultant, clinical consultant, supervisor, and testing duties, to people who are qualified for them. The director stays responsible.
