Common IQCP Mistakes and How They Show Up on Inspection

The most common IQCP problems are risk assessments copied from the manufacturer without lab data, missing environment or personnel risks, QC plans less stringent than the manufacturer, no director signature before use, and quality assessment that stops after go-live. CMS says weak plans usually surface as QC, QA or PT citations.

CMS says surveyors generally don't cite the director's risk decisions directly. They cite the downstream results (CMS IQCP Frequently Asked Questions (Aug 2016)). Here's where those results come from.

Mistakes we look for

  1. Manufacturer-only risk assessment. The CAP says manufacturer information can't be used alone (the CAP's public IQCP FAQ (updated Nov 2024)).

  2. Missing components. Environment and personnel risks are the usual gaps, as Westgard's outside review of a point-of-care IQCP found in a reviewed POC plan.

  3. No local data. CMS requires in-house data supporting the QC frequency.

  4. QC below manufacturer minimums. Not allowed by CMS, CDC or the CAP. That includes non-QC manufacturer requirements the reviewed POC plan left out.

  5. Data period shorter than the QC interval. The CAP says data must span the maximum interval.

  6. Unsigned or late approval. The director must sign and date before use, without delegation.

  7. One IQCP for different models. CMS requires separate IQCPs for different makes or models.

  8. Multi-site shortcuts. Each CLIA number needs its own IQCP and data.

  9. No ongoing QA. No monitors, no reviews, no CAP biennial reapproval.

  10. Using IQCP outside QC. The CAP says it can't reduce calibration or AMR verification.

  11. Running reduced QC with no IQCP. CMS says this is cited against the QC regulation.

How it gets cited

Under CLIA, surveyors use an outcome-oriented process. Problems appear as QC, QA or PT deficiencies. CAP inspectors can cite any missing IQCP element on their checklist. See CLIA inspection deficiencies and how to prepare for a lab inspection.

Common IQCP Mistakes and How They Show Up on Inspection checklist

  • ☐ Each risk assessment uses lab data, not only manufacturer data

  • ☐ All five components and three phases covered

  • ☐ QC plan meets or exceeds the manufacturer's instructions

  • ☐ Data span covers the QC interval

  • ☐ Director signature and date predate use

  • ☐ Separate IQCPs per make/model and per CLIA number

  • ☐ QA monitors active; reapproval current

  • ☐ Every reduced-QC test has an IQCP

CDC resources on CLIA

  • CDC's overview of the CLIA program: CDC explains that CLIA applies to every U.S. site that tests human specimens to assess health or to diagnose, prevent, or treat disease. It also explains that CDC supports the program alongside CMS and FDA.

  • CDC on CLIA test complexity categories: CDC explains how FDA scores tests as moderate or high complexity using seven criteria. It also notes that lab-developed or modified tests default to high complexity, and that moderate and high complexity standards differ only in personnel requirements.

  • CDC's free CLIA educational materials: CDC offers brochures and booklets on waived testing, PPM, and IQCP.

Frequently asked questions

Can a surveyor cite my IQCP?

CMS says the director's risk decisions generally aren't cited directly; resulting QC, QA or PT problems are.

What happens if I run reduced QC without an IQCP?

CMS says the surveyor cites the QC regulation you didn't follow.

Is a manufacturer-provided IQCP enough?

No. The CAP says it can supplement, not replace, the lab's own assessment.

Can one IQCP cover two analyzer models?

No. CMS says different makes or models need separate IQCPs.

Can IQCP reduce calibration?

No. The CAP says IQCP applies to QC only.
